2010 Summer Camp Information 

Here are the Registration Forms for Summer Camp.  Please print it and complete. 

All information needed for registration is found on the forms.  Each participant needs to fill out a form for each camp they plan to attend.  This means parents and grandparents attending Discovery Camp or Grand Camp with a child should also fill out a form.  Please give your completed form to your Minister or Youth Minister.  Forms must be signed by the participant, a parent, and your minister.

Please note that this form needs to be taken to all week-long camps to be turned in with the camper's medications.
